Continuity of Care

St Thomas Health Centre is trying to improve the continuity of care we provide patients. Continuity of care means seeing the same doctor/nurse regularly, rather than lots of different ones. This allows you to build a relationship with that clinician, meaning you get to know them and they get to know you.

Why should continuity of care matter to you?

Research tell us that continuity of care leads to better outcomes, including higher patient satisfaction, greater adherence to medical advice and decreased use of hospital services.

We encourage you to book to see the same doctor/nurse whenever possible please. We do recognise that this isn’t always possible, for example when same-day appointments are needed. If you regularly see a doctor who is not the one you are registered to then please enquire about changing them to be your registered doctor.
