Access to Medical Records

If you would like to make an access request please download the Application to Access Medical Records form and complete this fully. This needs to be returned to the health centre in person with 1 form of photo ID.

You may want to consider registering for online access which allows you to book appointments, view your medical records and request prescriptions.

Whilst we no longer charge for access to medical records, we may apply a small administration fee of 20p a sheet to a maximum of £5 where duplicated requests are required.

Access to Medical Records of Deceased Patients

On 1st August, St Thomas Medical Group became responsible for responding to requests for copies of medical records of deceased patients. Access to medical records is granted under the Access to Health Records Act 1990 (AHRA) and these records can only be provided:
  • to the patients named representative (executor or administrator of the will)
  • if there is a claim arising from the death of the patient
  • if we have been ordered by the court.
Please note that evidence of the above must be supplied along with proof of identity (1 x photo ID and 1 x proof of address). The AHRA does not grant access to records before 1st November 1991.
