Hypertension and Blood Pressure Measurement

If you are measuring your own blood pressures at home (either with your own blood pressure machine or one loaned from the surgery) please feel free to use our Form to document your blood pressure and heart rate.

Coronary Heart Disease

If you have Coronary Heart Disease we will ask you to have a review every year with our GPs and nurses.

Asthma

At each Health Centre a Practice nurse with specialist asthma qualifications is employed. They run clinics in order that asthma may be assessed, advice offered, queries answered and correct treatment ensured.

Patients on asthma medication are invited at least once a year to the asthma clinic for a check up with the nurse.

Asthmatics are invited to complete an online form which informs the asthma nurse how well controlled your symptoms are. Ideally this should be completed once a year.

Diabetes

At each Health Centre a Practice nurse with specialist diabetes qualifications is employed.

Our aim is to help you to manage your own diabetes and to help you to adopt and maintain a healthy lifestyle.

Patients on our diabetic register are invited to attend clinics which are held at St Thomas and Exwick Health Centres. A “drop in” clinic is held once a month at St Thomas where a specialist nurse will provide support and advice.
